How do they test for genital herpes when your preg with a blood test?

Yes, it is a blood test. You should get tested because it can cause blindness and/or many other problems in the baby if you have an outbreak during birth.

Skin tags don't go away. Herpes come and go. It's normal for your groin area to become irritated during pregnancy due to all the pressure and extra blood flow. Most ob/gyns do a full STD screening at the beginning of EVERY pregnancy, married or not. Bring it up at your next appointment, but I think you're worrying over nothing.

right....

skin tag is nothing to worry about, and, if you have EVER had any sort of internal it would have been mentioned if it was anything other than a skin tag.

As for red and swollen, its probably the start of thrush- which doesn't necessarily itch. testing for both herpes and thrush when pregnant is a simple case of taking a swab and having it sent off to the lab.
