Creams or ointment wont make the skin tag retract on its own. I happen to live in a backward town where the government doctors refuse to perform surgery on me as it is not health-threatening. And I'm not a candidate for rubber band ligation. If I perform the surgery myself under anesthetic, will I die of infection or worse, cut a nerve or artery?

just pull them out until you get them all out and cut them like an umbilical cord

4th degree haemorrhoids require surgery and ONLY surgery by a qualified General Surgeon. Self medication/surgery/going to quacks can be fatal.

Hemmoroids are called varicose veins of the rectum for a reason. Those are protruding veins you could cause yourself harm trying to remove a vein from your own body without professional help. Look for a private practice doctor that will counsel you on this matter.
